Nursing Team

Our Living Well Nursing Team specialise in palliative care. We provide expert advice and support to our patients who are living with a life limiting illness and to their loved ones.

Our Approach

As part of a rehabilitative palliative care approach, we offer advice around the control of symptoms that you may be experiencing and how these symptoms can best be managed. 

Our aim is to enable you to live the best quality of life that you can. 

We also recognise that an important part of living well is to support you and those important to you to make plans for your future.

This is commonly referred to as ‘advance care planning’.

We believe that part of living well is also about dying well too. 

Starting to contemplate an uncertain future can be difficult.  We are here to offer you and those who are important to you support; to talk with you all as you consider what matters most and to help you make plans that reflect who you are.

Assessment

We will initially meet you face to face and complete a holistic assessment to establish what the main issues are for you now and in the future.

We will then work with you to help achieve any goals that you have set yourself through our therapeutic group programmes.

We also offer individual support and advice.
