How to seek a referral

We care for people with different types of advanced serious illness or life-limiting conditions including cancer,  heart conditions, lung disease and also neurological conditions like Motor Neurone Disease (MND), Parkinson's and end stage dementia.

If you feel you would benefit from one of our services, please discuss this with your GP, District Nurse, or other health or social care professional. Patients cannot self-refer.
